Job Description

The Paulist Fathers, a missionary society of priests, seek a Registered Nurse to serve as a Professional Health and Wellness Coordinator primarily for their Motherhouse, which is located on Manhattan's Upper West Side. The successful candidate will apply knowledge of nursing, leadership skills, and administrative experience to plan, organize, develop, and oversee the health care of The Paulist Fathers within the framework of the teachings of the Catholic Church. Key Responsibilities Include: Developing and implementing policies and procedures that adhere to the scope of professional nursing practice, using evidence-based nursing, National Safety Patient Guidelines, person-centered care model, and other quality standards to guide development. Managing medical appointments, transportation and medication. Accompany Paulist Fathers on medical appointments as requested. Conducting regular health and wellness check-ins, initiating activities to promote wellness of body, mind and spirit, and providing recommendations based on analysis of data. Engaging and supervising additional staff as needed. Coordinating with other staff of the Paulist Fathers regarding Medicare and Medicaid services. Identifying house needs for senior and infirm Fathers. Facilitating referrals to care facilities. Researching local short term and long-term care facility options and building relationships with the community and its resources. Acting as a resource for other Paulist Houses when requested. Maintaining Advanced Care Plans. Working in conjunction with health insurance providers to coordinate, maintain, and secure the best care available for each Father in the most cost-effective manner possible. Contacting appropriate individuals to be involved in an assessment of a Father's medical condition and needs. After hospitalization or treatment, reviewing the treatment and prognosis of the condition to assist with appropriate aftercare. Working in conjunction with health insurance providers to coordinate, maintain, and secure the best care available for each Father in the most cost-effective manner possible. Experience: A Bachelors degree in nursing (BSN) and current NY state license in good standing. Current CPR/AED First Aid Certification. A minimum of 5 years of clinical experience with adults, preferably with a gerontological population. Compassion, warmth, and capacity to recognize in the aging and infirmed a gift full of wisdom. Proficiency in policy development and writing. Basic knowledge of healthcare procedures, insurance, and social benefits (federal and state). Knowledge of community resources, such as local long-term care facilities, and the ability to develop relationships with community resources who offer these services. Demonstrated commitment to continuing education relating to the following: health and wellness promotion; care of the elderly; management; measurement and evaluation of health care quality and safety outcomes; team building; conflict resolution. Ability to travel with, transport and escort Paulists during medical and resource-related appointments; Drivers license and safe driving record. Proof of current liability malpractice insurance. Successful completion of a background check. Experience serving as a health care proxy decision maker and health care advocate for others, preferred. Understanding and experience working with religious communities and how they live and operate in community, as well as the pastoral nature of the role, preferred.
